I hereby give my consent as the parent/guardian of the above named participant to attend and participate in Easter for all hosted by Peace Lutheran Church at 200 E. Beidler Road King of Prussia, Pennsylvania. All who attend hereby release, indemnify, and hold harmless the church, its employees, and/or its volunteers from any and all liability from any claim, injury, or loss sustained by or during participation of Easter for all or any events hosted by Peace Lutheran Church.
I hereby authorize Peace Lutheran Church to take and use photography and/or video of all who attend Easter for all for crafts, keepsakes, or promotional purposes in any type of media and understand I will not be compensated for any such use.
